If you would like to file a complaint in reference to a food establishment, failing septic system, mosquitoes, and/or any other general sanitation concern, please complete the form as accurately as possible.

WARNING:  Any personal identifying information that you place on the page is discoverable upon written request by any citizen, as a result of the Open Records Act of the State of Texas.  If you wish to remain anonymous, do not put any information under the complainant’s information section.  Please be assured that anonymous complaints are investigated as aggressively as complaints that identify the complainant.
